Clinic Appointments

Scheduling your appointment

Some patients may self-refer for consultation appointments.  Please call us if you're unsure if a doctor referral is required.  To schedule a clinic appointment, please call (773) 702 - 6514.

If you need to reschedule your appointment, please call us at least 24 hours prior to your visit.


Preparing for your appointment

  • For imaging studies (CT, ultrasound, MRI, etc.) done at another facility, please have available what imaging study was done, where, and their contact information. We will instruct you or the facility on how to send us the images (usually a CD) and the radiologist's report. It is important that we have the images available to review at the time of your clinic visit.
  • Bring your insurance card
  • Bring contact information for your referring doctor, primary care doctor, and any other doctors you would like us to stay in contact with regarding you care.
  • For lab tests done at another hospital, please ask them to fax us a copy of the results.
  • Plan to be at the clinic for 1 hour.

If you are coming from home you will check in at the Sky Lobby (7th floor) of the Center for Care and Discovery approximately 15 minutes before your appointment.  After you check in with the receptionist, you will be escorted to the Interventional Radiology Clinic on the 5th floor.  Here you will be greeted by one of our nurse practitioners.  You may be asked to fill out a brief medical information sheet (medical history, medications, allergies, etc.) before you are seen by the doctor.
